The core difference can actually be summed up in one sentence - physical examination is to passively find existing health problems, while preventive health care is to actively prevent health risks that have not yet occurred. The routine physical examinations we do every year, including routine blood draws, chest X-rays, and ultrasounds, are essentially taking a snapshot of your current health. They can only detect problems that have already caused abnormalities in physiological indicators or even have formed lesions. It is equivalent to giving an instant score to your physical condition. It cannot cover changes after this point in time.
I have been doing health management in a community health service center for almost 6 years, and I have encountered too many people who let down their guard with a perfectly normal physical examination report. Then they turned around and found out that they had gout caused by continuous overtime drinking and heavy drinking. They were in so much pain that they couldn't get out of bed. Preventive care just fills this gap. Take the 68-year-old Uncle Zhang who was followed up earlier. His fasting blood sugar during the physical examination at the beginning of the year reached 6.3mmol/L, which is just on the borderline of abnormal glucose tolerance. We did not wait for him to develop diabetes before intervening. First, we replaced the whole grain porridge he drank with glucose-increasing sugar. He was fed slow multi-grain rice, and was reminded to go downstairs and walk for 40 minutes every day in the community group. He was also asked to test his fasting blood sugar at home twice a week. After 3 months, the indicator returned to within 5.6mmol/L. His physical examination has remained stable until this year. This kind of long-term, personalized pre-intervention is the core of preventive health care.
Of course, many people now think that preventive health care is an IQ tax, and regular physical examinations are enough. This kind of doubt is actually not unreasonable. There are indeed many unscrupulous merchants on the market who sell sky-high health care products and provide useless physical therapy under the banner of "preventive health care", which has ruined the reputation of the entire industry. However, formal preventive health care is all evidence-based. From vaccination, intervention of high-risk factors for chronic diseases, to daily diet, exercise and sleep guidance, each item is supported by clinical data and is not a bluff.
To use a very common analogy, your body is like a family car that has been driven for more than ten years. The physical examination is an annual inspection. Check whether the brakes are good, the exhaust gas is not passing, and whether any parts have been worn out. If there is a problem, a red light will be turned on for you to repair it quickly. Preventive care is your usual car habits. Add the appropriate grade of oil, change the oil regularly, and do not slam on the accelerator or brake. Try to reduce the loss of parts and extend the service life of the vehicle.
In fact, the two are not completely separated. The physical examination itself is a key part of the preventive health care system. It can help us detect early abnormalities that we did not notice in time and point out the direction for subsequent health care intervention. If you really want to avoid going to the hospital, don’t bet all your money on the annual physical examination. Spend more time on daily health management, which is better than anything else.
